Dissertation Forms

A form documents the successful completion of each stage of the dissertation (proposal defense, final defense, completed dissertation). Students are required to bring the form (e.g., to the proposal defense) and to obtain the necessary signatures.

The completed defense-related forms must be submitted to the PhD program director (or his or her designee) as soon as possible after the completion of each defense. The completed Certification of Dissertation form is submitted to the University with the dissertation itself.

The PhD degree is given by the University, not the School, so University-wide requirements for the dissertation must be followed. Deadlines and other information about writing and filing a dissertation at the University of Pennsylvania is available on the Office of the Provost website.
